Those banshees from Mass Effect 3. You would hear them scream long before you saw them, and they moved like the sci-fi version of the girl from the ring. They were were bullet sponges, and if they got too close, you were gruesomely instakilled.

The Witch in Left 4 Dead. Her cries are creepy as heck, but when you're in her sights it's downright terrifying.

The very first Halo when the flood were introduced for the first time. It was pretty intense and I don't remember anything like it in a video game at that time.

I have to second this. I had no idea about the flood before they were introduced. 13 year old me had issues shaking of the feeling of being attacked by balloon sized parasites that night.

Probably Marathon...Bungie did some amazing level design in the game, and also used sounds – soundtrack, as well as ambient and triggered sound effects – to great effect. To this day, I still feel just a bit creeped out when I call the "Hunter activation" sound effect to mind.

Alma from the FEAR series. Completely. Had me weary walking around my house in the dark after the first game, still makes the hairs on the back of my neck stand up!
